You dispute with the escrow normally. Personally I've never had it happen, but you would have to talk to some of the known escrows here like OgNasty, Minerjones.

You can also do shipping with signature confirmation for packages. I recommend having a tracking number that can be seen by both buyer and escrow to follow the delivery.
