After trying some things out myself, I think I have come to the conclusion that your trust settings are what is hindering you to place feedback on Blazed for the same reasons as I mentioned earlier.
If I copied your Trust List and set my depth to 4, it would not load the page fully and wouldn't show the form. Is this the setup you currently have?
Either way, if you temporarily remove your trust list and set your Depth to 2 you should be able to leave him the feedback that you want to.
I think that a decent server side solution for this would be to cache the trust pages of users with a large number of feedbacks so that it didn't have to call it from the database every time. This would probably lower load times and solve issues like this without compromising any sort of security measures.
