I have exactly the same problem.

But bank account infos are not something really private. And it might have been stored in the database. As such, there's no information I could give to MtGox that isn't public.

Might also be that I've changed my password on MtGox before the hack. I was thinking about it and always assumed that I tried but that it didn't worked (bug on MtGox). What if it finally worked? How can I proove that I'm the one requesting the account?

another rejected request Cry though I provided LR account number and exact BTC balance.

Today I reapplied. Provided the same info plus IP and last trade info also had signed up to Mt.Gox Support Desk and wrote them an e-mail providing a .pdf file of my e-mailing history with Mt.Gox admin containing the request for withdraw limit increasement and the pics of my passport.

Will feedback on the results!

I provided my withdraw account details, my static IP and my account balance (rough). I had no problems.

Did you try to contact them? I opened a ticket and i got an answer within few hours.

Well, I finally got accepted. Don't know what changed because I didn't provide more informations except saying I was not sure of my balance and giving a range instead of a number. Seems that it was enough.
