I'm using CGMiner 3.1.1. I've got a Satechi UH-12P 12 port usb 2 hub, with an upgraded 5 volt, 6 amp power supply. When I run CGMiner, everything works fine for a few minutes, then 3 of the erupters will randomly start giving errors:
ICA1: invalid nonce - HW error
ICA0: invalid nonce - HW error
ICA3: invalid nonce - HW error
Not in that order though. It's pure random.
Here's my setting:
cgminer-nogpu.exe --icarus-options 115200:1:1 --icarus-timing 3.0=100 -S COM3 -S COM4 -S COM5 -S COM6 -S COM7

I had the same issue with my hub when I first got it. I changed a few things:
1) --icarus-options 115200:1:1 --icarus-timing 3.0=100 -S \\.\ COM5 -S \\.\COM12 -S \\.\COM13
note the "\\.\" before the com ports. Shouldn't make a difference with the single digit com port but it was causing issues on my end.
2) Move the usb erupters around. I have a 7 port 2A hub and I found that I needed to space them in a certain configuration to get them to draw power correctly. If you don't know which ports are wired in series with which others and which ports are wired in parallel with which others, you're going to be guessing and checking for a bit. But while 6 amps SHOULD be enough to run every port, sometimes the power draw is a bit screwy and you'll need to space them differently.
Common power distributions will group the USB ports with power supplies like this (Assuming a 12 port hub like yours):
plan 1: AA BB CC DD EE FF
plan 2: AAA BBB CCC DDD
plan 3: ABCD ABCD ABCD
plan 4: ABC ABC ABC ABC
