The main reason behind unsuccessful trading is Trading w/o any plan. If a plan is made before taking a trade chances of getting higher success rate becomes brighter.

Planning of a trade includes:

1. Check the higher trend, where is it heading to.
2. Check the middle trend.
3. Now come to lower TF to decide your entry point, there should be not resistance/support where you are taking a trade, wait for the right setup.
4. Where is your exit point, if trade goes in your favor where would you like to exit and where is your SL in case trade goes against you, that should be pre decided.

If one follows such simple rules, chances of success get higher.
