Do we have any thread where we can report ban evasions so that mods can look into the profiles and ban if appropriate?
As ar as I know there is no any such as thread. You may create a thread regarding ban evasion on Meta, so that moderators will take look and could take appropriate action. Don't forget to attached appropriate evidence.
Tagged for now, hope they will get ban very soon.

There is but don't know how effective that thread is, just remember Stherapist created a thread on that back in early January;
Ban Evasion [Requesting Admin/mod to check those and ban] you can use his thread as a reference point to highlight all the needed evidence to validate your report, copy the link to your reply and report to moderators or simply do that on this thread and direct the moderator to your reply via a link reference.
I think it's better to create new thread since OP of your mention thread's is inactive for long time. Or better create individual thread either.
