One thing I'm still curious about: if both accounts were KYC-verified under the same identity, would 500 Casino's KYC/account systems normally detect that the same person already had an existing account?

While I'm not a support staff, they have mechanisms in check or conduct manual reviews just like how other casinos operate. Ultimately we strive to not see this happen (unfortunately we can't control whenever a player tries to scheme).

In other words, even if Rioluth never received explicit permission from support, was the second account identifiable as belonging to an existing customer at the time it passed KYC, or is that only something that would normally be detected later during a withdrawal or sportsbook review?

It might have gone through either of these:
