You can see all the staff members here: https://bitcointalk.org/index.php?action=mlist;sa=search . Check only the search by position  box and in the box type staff for all the staff members or global for all the global moderators. You can't see each moderator's powers (not even I)(In some cases under the description of a board, it says Moderators: ). Some moderators have moderation privileges in many boards or members, while some others in less boards/members.

Mitzie is also a mod and I think there was a couple of new mods in one of the local boards recently.

There's no comprehensive list anywhere but you can see Members labeled as Staff here: https://bitcointalk.org/index.php?action=mlist;sort=ID_GROUP;start=323190 (scroll to the bottom). Global mods and Admins wont show up on there though but you can find them easily enough.

Check the Members page in the menu.
Example: https://bitcointalk.org/index.php?action=mlist;sort=ID_GROUP;desc;start=30 (check the next page as well + admins are first in the list, so they don't appear as staff).

Really depends. Sometimes they're just added when a member actively and accurately reports messages. Some when certain changes happen in the forum.
