No, only 1 fan is working. The one reading 720 is only spinning because of the other fan forcing air through it. Usually it is the exhaust fan that dies.
You are right, but it is not always the exhaust fan that dies. In my high corrosion area the intake fan fails maybe 4 out of 5 times. The higher numbered fan sockets are closer to the edge of the controller board. Just follow the wire from the socket to find out which fan you need to replace. (So in your case, fan[3] is lower and its socket is farther from the edge of the board).

You can use bmminer-mix to recompile bmminer.sh as you like. I wouldn't go much below 8.4V. The definitions you need to edit are around line 588 of https://github.com/bitmaintech/bmminer-mix/blob/master/driver-btm-c5.cI think it is not good to use some custom firmware because you don't have a chance to recover or reflash it back to its original firmware.
If you do what I've just described all you have to do is replace bmminer.sh, you can make a backup of the old one and restore whenever you like. Or, you can always go the upgrade section and flash it new again.

I see this issue almost daily. Usually rolling back the firmware will let you run while ignoring the issue.
If a board is zero you can replace the PIC with one from a spare board if you want to. Bitmain doesn't seem to sell spare PICs.
I have also seen the issue happen because of a faulty ribbon cable. Try replacing the ribbon cable to the board giving you the issue.
If you get the 413 error when flashing firmware, use a microSD to flash manually.

Try running one hashboard at a time and see if the issue still occurs with each one. Could also be a power issue, have you tried another psu?

Probably something wrong with the hashboard.
You can always try using the package to fix fw upgrade, flash it to SD card, move the jumper pin and flash using microSD. (Flashing the april firmware of course).

You know bitmain is selling S9i 14.5th for only $414 and with PSU for $499? With coupons they are even cheaper.

can this be modified so you can pick the frequency that works best on your particular miner or using a lower watt PSU? for exa,ple instead of 17TH at 2,300 watts can this be clocked lower to run say 15TH at a lower power draw so it can run on an 1,800 Watt PSU?
In bmminer-mix you have to define BMMINER_VERSION to a fixed frequency option and upload the new bmminer.sh. After that, you can use minerAdvanced.cgi to set the frequency to anything you like.
