Yeah, I have seen multiple persons saying they lost all their money 15 minutes after they received it. That's exactly why I instantly withdraw my earnings after I receive them in my stake account, because the temptation is to high and if you leave it there eventually you will use it.

On big casinos (some of them) you'll see competitions and promotions regularly it's optional but can be effective. The difference is that on small casinos the prizes are smaller and only happens once or twice. Got 0.00000100 ETH from fauced. Made a 0.00000005 ETH autobet and went for a lunch. When I came back I had about 0.00000265 ETH. Changes the bet to 10 and switched to read forum. Now when I look at the balance, it is 660 already

Offering free withdrawals wouldn't be very good for Stake and that's because they process hundreds, maybe thousands of withdrawals daily. The fee is very low and at first it wouldn't seem to affect them to much but when you think about the thing that they process tens of thousands of transactions monthly and do a quick calculation they would lose a lot of money just for fees. There is a possibility of increasing their house edge but offer free withdrawals in exchange for that but this would completely change the structure of the casino so its not a good idea. The fees are extremely low anyway, being about 0.2$ right now so I don't think this would be a concern for anyone.
