Check this page: https://iancoleman.io/bip39/

For security you may try offline or download sources from git.

On the page you may convert your seed to private key, and then you should be able to import it to any client (like electrum, etc)

Never heard that there is a wallet that could generate 6 words seed phrase.

Anyway, can you try to install Electrum and enable the BIP39 option it might be work?

If not, the only option that I know is by using the iancoleman as suggested above

Coinomi wallet is another option I just found the idea from this link below.

Check this page: https://iancoleman.io/bip39/

For security you may try online or...
Wrong piece of advice. OP should use the tool offline if they are security conscious.

You can use less words but it's not as secure.
Maybe you can check out different word lists to see which list was used to create the Mnemonic.
Here is the Bip39 wordlist https://github.com/bitcoin/bips/blob/master/bip-0039/english.txt
If it's not that one then check out this one that was used in the past by electrum: https://github.com/spesmilo/electrum/blob/master/electrum/old_mnemonic.py
Depending on the method used you can turn the seed into a keypair.

Go to iamcoleman site
Enter your 6 words in  "BIP39 Mnemonic" field
Check tabs below, in one of them there should be your address, probably bip32.
If you find corresponding address, you may use value from column 'private key" to import into Electrum wallet, or any other software. Or even into blockchain.info web wallet (but do not do it).

So it's some sort of paper wallet? I'm curious: how did you make/get it? I've never heard of it before.
If the words aren't standard words, it might be a brain wallet. When you figure it out, don't forget you have about 5% in Forkcoins. But secure your Bitcoin first.

Are there QR Codes in the "redeem" boxes that you have blanked out? Huh

If there are, then you should try and scan them using a wallet like Electrum that supports QR code scanning... you can use either the Desktop version (with a webcam) or the Android version.

When you create the new wallet, you select "Import Bitcoin Addresses or Private keys" and then select the little camera icon:
Desktop:

Android:

It's possible that the 6 word "passphrase" you have is actually a BIP38 passphrase, encrypting the private key... so, if you scan the "Redeem" and it comes up with a key that starts with "6P", then you'll need to decrypt that key...

I would recommend downloading an "offline" copy of bitaddress.org (or visit their github: https://github.com/pointbiz/bitaddress.org)

Then on the wallet details page, enter the BIP38 encrypted key (starts with 6P) and click view details:

It will prompt for the "BIP38" password:

Enter the 6 word passphrase from your PDF and click the "Decrypt BIP38" button... and hopefully that will decode it and show you the "normal" unencrypted private key:

You can test the BIP38 decryption process using this dummy BIP38 encrypted key:
BIP38 encrypted key: 6PYRJRTWwrPC7scdAPcWN9dujLRKVdMRYuKmhXuefzQGKXvqNe8Gjo7uLQ
BIP38 password: This is my password

It should decrypt these addresses/private keys:

Address: 18KmgRWoh2zYF7fBWUVzxqiipDNCoNAXEA

Your image isn't working.

I can help you better if you answer those questions:
how did you make/get it?
If the words aren't standard words, it might be a brain wallet.

The point is that we also do not know ;-)
There are 2 possibilities:
1) What is called 'passphrase' is a 6 words seed - it is possible, but you must verify it yourself; I told you how to do it - You will have to convert it to format 'importable' by current wallets.
2) "Passphrase" is a sentence used to generate 'brainwallet' - which means that it could be any words, any sentence -> and it is converted into private key.

What you were asked before is to check what are the 'words' in passphrase - is it a sentence in English, or just a 6 words? Someone mentioned a list of allowed words:
https://github.com/bitcoin/bips/blob/master/bip-0039/english.txt
Are your words on the list or there are others?

If you confirm that words are not from the list, that it is more complicated.
What you may try, is to use is to generate 'brainwallet':
https://brainwalletx.github.io/
If you use your sentence on this page, do you receive expected address (in compressed or uncompressed mode)?

Bip39 is BASE2048.
Electrum used BASE1626 up to 2015.
You can use both protocols with a 6 word mnemonic but it's not recommended since its only 64Bit security.
Did you also check that word list ?
