As far I remember, there are few schemes defined by constitution and federal laws on self-governing. Appointment by the president with approval by the local parliament, unanonimous election by the parliament from the set of offered candidates, unanonymous election by the local parliament or anonymous election by locals of district [city, republic or whatever]. Exact scheme is defined by the regional statutory document.
Right now ER has a majority, of course, but there are communist and LDPR governors, mayors, MPs... Just one example, mayor of Novosibirsk (3rd largest city) is a member of communist party.

ER supports him, but it's not "his" party. It was founded by Boris Berezovsky, a dead friend of Mikhail Khodorkovsky, and funded by him from the beginning... Many people still think that large stake is still owned by his cronies. So I'd say that it's a party of oligarchs rather than Putin himself.
