I assume you'll be doing a sendmany transaction, John?
And Ben, I realize that anyone could fill out my form. This is why I wanted John to double check it in the end but it, like your form, got things a bit more organized. It didn't quite make it to generating a sendmany transaction from the results (which is what I wanted to accomplish), but between the two of us I would like to think it made John's job just a little bit easier.
Anyway, let me know if I can do anything else to push this along. One good thing has come of this whole ordeal - we know who has their heads up their asses and have found some more people who weren't known before who can actually pull some things together and may be good people to transact / work with in the future.

that's nice and all, but this is a one and done event (the refund) so the time it takes to put together a script would be the same amount of time or more than just sending them out one by one

Why is anyone against doing this? Don't you realize that this type of transaction is done all the time with mining pools, gambling sites, etc? It's not a script. It's a command built into the bitcoind protocol / API. Not only is it faster (eliminating doing each one individually), it also applies the mining fee to the one transaction (for everything) and not to each individual transaction. It doesn't take any time to put the command together. You can do it yourself and see. Open John's spreadsheet and then save a copy. Take the first bit of code, paste it into cell F2. Then select that cell, go to the bottom right corner where the plus sign shows up, and drag-copy it to all the cells below it in the rows that have content. After that, the second bit of code and paste it into G2 and press enter. Voila - you have the sendmany transaction with one extra comma. Copy the contents, remove the comma before the closed curly bracket, and press enter. Everyone's funds are sent in one transaction. This also saves time because only one transaction needs to be linked to everyone. We can see everything sent and John doesn't then have to copy / paste the transaction to every person - a time saver I neglected to mention initially. Come to think of it, for as long as John has been doing escrow, I'd be surprised if he wasn't already familiar with sendmany. He might have already even used it.

Some of us are on edge because we have thousands of dollars in this and we've been going around in circles for weeks with different forms, confirmations, people gathering them, adjusting for chip transfers, people trying to scam/break said forms, etc. maybe a send many is fine, but the last fucking thing that should happen is he finishes up this form and sends it out immediately

After weeks of collecting information, when exactly would you like this money to get refunded? I'm not clear on what you want to happen here.
