To be approved, you have to be a credible source of information. It is done on a case by case basis. For example a government is a credible source of information, so databases such as NMVTIS will be approved contractors

The hashes will confirm the immutability of information. For today there is no possibility to store all the information in the blockchain. But as soon as technology appears, we will go further.
