Whereas on Coingecko P2PB2B is ranked 114, Does coingecko know if it's bullshit exchange? so it's not in the top 3 like in coin marketcap

Until the same update in coinmarketcap, I will follow CoinGecko website to use any exchange. But for tracking any new or old coin, I will follow the coinmarketcap website. Of course the coinmarketcap is better than CoinGecko. After the next update of coinmarketcap, the exchange ranking process will be the same as CoinGecko.
