If you'd like to get a BIT-X debit card you can either pay a setup fee when it's released (0.05BTC, you'll still need to complete basic verification to get the card) or complete basic verification now and get the card for 0.05BTC less when it comes out.

To get basic verification, fill in the form, enter your personal info and your living address and attach the documents confirming the information provided.
NB! Prior to sending the request make sure that the documents confirming your personal info are fully scanned in high quality (not less than 150dpi) and the data in them coincides with the data you provide in the form.
Please use only latin alphabet letters to fill in form fields.

I have both sms and email selected for  notifications and have received my payment notifications on both.

However I have not received the debit card offer notification via text or email.  Not a big deal for me but I imagine others may have the same issue.
