Not sure what I am missing, but what screw are you using to attach the Nepton 280L cooling head to the board, nothing seems to fit.

I used the shorter set of screws that came with the Habaneros, combined with the little black plastic spacers from the Nepton 280 Kit.

Still waiting on a couple larger orders to come through. Unfortunately it's not economical to do a run of 100 boards, and we don't have the several hundred thousand dollars needed to do a reasonable sized run. This has definitely been a slower process than we would have liked. Regardless, if it does go ahead we should have plenty of notice before boards are ready.

I had to go to home depot and get 8x 3/4" 6-32(think thats what they are, well normal coarse thread PC) screws

Same here. I actually prefer this method to the thumbscrews, have more control and easier to make small adjustments.

I measured all the pcie connectors.. they are all 12.8v.

But I noticed that FAN 2, 3 has low power as the fans are slow, FAN 4 has full power (fan is going really fast)..

That's normal, FAN4 and PUMP run at 100%, they can be used for your pump or something like a chassis fan you want powered at 100%. FAN2 and FAN3 run at 50% on startup, until instructed by the mining software to change speed.

I have seen "HFA : OP_USB_INIT failed! Operation status 20 (Regulator programming error)" and I think it's lack of available power that can cause this.  If you could monitor the voltage of the PCI-E while running you might see if it is dipping during operation.  It is normal for FAN 4 to be fast all the time.  My board does the same.  The other fans speed up once the board is mining successfully.

I have seen "HFA : OP_USB_INIT failed! Operation status 20 (Regulator programming error)" and I think it's lack of available power that can cause this.  If you could monitor the voltage of the PCI-E while running you might see if it is dipping during operation.  It is normal for FAN 4 to be fast all the time.  My board does the same.  The other fans speed up once the board is mining successfully.

How can I monitor it during operation?

I am using gigampz with the hp server psu (1000w). All 4 pcie plugs according to my multimeter are operating at 12.8v (not plugged into the board).

Use a volt meter if you have one.   You could try running at super slow clock rate.   100Mhz for example .

I used a volti meter on the plugs themselves (not plugged into the board) and they are all fine.

I tried it a 100Mhz and upon starting cgminer it does the same, it within a millisecond the fans/cooler and board seems to power down/restart or something.

if you can flip the board over while it's running you'll see the pins for the PCI-E.  one of the rows is +12v and tthe other is ground.   Check those while it's trying to power up.   It might be difficult to spot so watch the voltage carefully while you start cgminer.

Just done that, while cgminer is running, flipped the board and used the check the voltage.. ALL 12.7v, no changes in volts.

When the cooler, fans are powering down and coming back on, is that the board resetting? (the 8 led strip cycles too)

Could it be a cooling issues? Seems strange to trigger it only milliseconds after starting cgminer (hasnt even logged into the pool yet to do anything). It only happens when starting cgminer.

if it has a cooling issue it could do this.   try 25Mhz clock rate.

Still waiting on a couple larger orders to come through. Unfortunately it's not economical to do a run of 100 boards, and we don't have the several hundred thousand dollars needed to do a reasonable sized run. This has definitely been a slower process than we would have liked. Regardless, if it does go ahead we should have plenty of notice before boards are ready.

We're having the same problem with getting DPS-2000BB PSU boards to production, large customers with the foot-dragging. Hopefully everything works out for a batch 3 for you guys. Gonna want one of these to play with.

Not sure what I am missing, but what screw are you using to attach the Nepton 280L cooling head to the board, nothing seems to fit.

I used the shorter set of screws that came with the Habaneros, combined with the little black plastic spacers from the Nepton 280 Kit.

Batch 1 thing maybe.  $250 more but came with socket cap screws.  Cost was easily offset by a similar amount of mining profits and having fun sooner than everyone else.   Grin

As for your trouble - the only time I've seen the regulator programming error is when there is an issue with power.  

Have you tried a different PSU?  What's your mining platform?  What's your commandline to launch cgminer?

Have tried a different PSU, but same gigampz breakout board (will try another breakout board now).

Mining platform is Ubuntu running cgminer 4.4.1.

did you try the 25MHz clock rate?
