I have some questions regarding your network:

1. Is it OK to use "rotating" ad slot?

pseudocode:
if (random(1,2) == 1):
    display_mellowads_ad();
else:
    display_other_ad();

2. We're displaying some ads in a member-only area of the website. Do you want a login/password for verification, for your staff or your bot?
3. All 3 ads we use are still marked as "unverified" on your site. Is it OK, or shall I be worried?

1) Yes thats fine so long as it is being done on the server - not on the client. If you are loading the ad in the browser then hiding it then that is not allowed.
2) No thats fine
3) Don't worry about that. That is to indicate that we have human verified the location and details of your ad space, but we are a bit behind with doing that. It won't affect your ads displaying or earnings.
