It gets paid out in 2 chip increments every time you hit a specific rake target. I forgot what that target amount was exactly, though. You will see how much of your bonus you have received in your wallet > Poker Bonus Chips Freed: XXX

And it is added directly to your Poker Balance Total.

You get the 50% + your VIP rakeback on top of that. So for example, if you are level Silver III, 20% rakeback and you start a table, you get 70% rakeback. You have to play sit the table continuously for at least 60 minutes to qualify. If you leave the table before sixty continuous minutes has passed, you will lose the bonus and only get your regular VIP rakeback rate.

I believe this is how it works. Someone please correct me if I'm wrong.

It is 50% more.
You get your vip rakeback and 50% more.

At the bottom of  that page, after a detailed axplanation of the steps you need to do to be entitled tablestarter rakeback, you may read:

"You will also automatically receive the rakeback from your VIP status on these tables which is a minimum of 10% and up to 50%. The combined rakeback for the play on tables you start is 110% to 150%."

That link is not updated with current tablestarter rakeback percentage (it used to be 100% but they changed it to 50% lately) but that percentage change didn't affect the way it works.

Also had a chat conversation with betcoin support that confirmed what I just wrote.
