Before Bitcoin was invented I wondered, why isn't there a egold to cash card? It's a global market I thought, surely a bank in Panama etc could offer this?

Which you can load using funds from either Liberty Reserve or using Mt. Gox USD redeemable codes / vouchers.  Using MTGUSD there is a 5% load charge.

Mt. Gox's CEO has said Mt. Gox will be issuing a Bitcoin branded prepaid card as well, though no dates or other details have been disclosed yet, as far as I know.

Don't know any more about it, myself.  I suspect if involves figuring out who to speak to and being able to guarantee a certain volume to make it worth the issuer's while.  Oh, and capital.  Mt. Gox bought an embossing machine to issue the cards, I believe.

So, I restored an earlier wallet backup (from when I still had the spent coins) and resubmitted the send request.

Just to clarify, .. you didn't resubmit the send request.  Instead you sent another transaction of the same amount as the earlier transaction.

So now that you have a double spend attempt in your wallet, you'll probably need to restore that earlier wallet yet again and it will then properly show the balance and the one transaction that did confirm.

The Mt. Gox redeemable code / voucher api is used by the open sourced MtGoxLive Trader mobile app. 

The voucher can be issued for any BTC or USD denomination.

I was going to give a link to the Bitcoin-Trader project on GitHub, but I see that it is no longer published.

Ok, you want to accept payment on 0/unconfirmed but you then want a guarantee against a double spend?

Unfortunately, the choice will boil down to either you accept the risk or you wait until one confirmation (or more, depending on the type of transaction).

ONE of these sites will adopt Bitcoin, because they are in intense competition with each other. The international/cross-border dynamics of these services is perfect for Bitcoin. I imagine these companies are watching the Bitcoin space to see how it develops. When it's more mature, they'll be racing to jump on board before their competitors.

The reason you don't see it often is because trading bitcoins for PayPal is against their User Agreement.

Mining is still profitable at current price/difficulty.  There is no reason for anyone to stop mining unless the price drops even further.

Include cooling costs, ... or consider that electricity at its marginal rate can be over $0.20 per kWk and over $0.30 in some areas, and you'll realize that mining truly turns into a money losing proposition at these levels for many.

If you are pasting the PIN instead of typing the entry, perhaps the paste includes characters other than just the PIN?

And of course it makes the password have to have a capital letter, which isn't any safer, just is a pain.

Lowercase passwords are cracked faster (and thus, less-safe) than a mix that includes uppercase.  

May I suggest having KeePass create a strong password for you?

And after registering, the same password that I just entered 50 times suddenly does not work because the website is sadistic.  So I have to do password recovery to be able to log into my account.

If the site keeps "forgetting the password," it is quite possible that someone else is resetting the password and that you just don't know about it.  i.e., if your e-mail is compromised, then whomever has access to your e-mail can oftentimes get the password reset on another site fairly easily.  

With Dwolla if the adversary knows your Dwolla ID#, Ph# and D.O.B. and has access to your e-mail those are all that is needed to first reset your Dwolla PIN and then to reset your Dwolla password.  The adversary would then hide the evidence by deleting the e-mails that those steps produced.

And the next time you attempt to log in to Dwolla, boom -- the old password no longer works.
