Well, ICOs fail due to a couple of familiar reasons. One of which is the poor project conceptualization and execution. There's a few factors why ico's fail. The team, the project and the community. Either the team is not committed enough to deliver the ico project, nor the project fail to deliver and have real usage or the coin/token was not widely accepted by community. And let's not forget that there are scam ico's which are bound to fail.
