Has anything similar happened to any of my fellow managers? This is not the first time that I made a substitute and it was accepted and updated, then afterwards when the scoring was done I noticed the player was still on the bench. This is extremely frustrating as I could have had an extra 13 points on top of the 70 that I had accumulated.

When did you do it? The deadline was 1,5 hours before the Wolves - Liverpool game on Friday.
If you did it after the deadline, you're assembling your team for the next gameweek.

Not sure if I'm explaining something you already knew - in that case, it never happened to me that an accepted change was not applied.

I only realised after the game ended that Timber was still on the bench and that was when I realised it has happened to me a couple of times in the past.

Never happened to me in all the seasons I played. When I am doing last minute transfers/changes I just switch from team page to transfer page and back. If everything looks all right it always is all right in the end

As others have not complained about this before, it most probably is the way I navigated the pages. I think I have to make the selection and refresh the page or at least use a different page and then return to ensure the substitution worked. I hope to use a chip next week to at least try to move up in the table, I am lagging behind.

If you're using the FPL website to make your substitutions, you must use the " Save team" at the bottom to lock your team selections for the week. If you fails to do this, it would revert to the old team after you leave the page.

If you're using the app, it's works automatically.

I appreciate the clarification. I am not using the app, I never knew they even had one 

On the website I always use the save button after making substitutions. I noticed they were sometimes not saved therefore began browsing another page before coming back to the page. If I did not find the team as I wanted, I would complete the process all over again. The next time I make changes I will not leave the website until I am satisfied the changes have been accepted.

Also remember, substitutions made after deadline will not work for the current week.

You are right, it seems I am the only one having this problem. I will keep a closer eye on my teams before the deadline to ensure I have the players positioned as I wanted. Hopefully whatever went wrong a few times in the past will not go wrong again.

Keeping that aside, I am seriously considering  changing my formation to have five at the back. I have tried other things and they do not work therefore it might be worth a try

Anyway GW 29 winner is Kanapka, on tie break. Very respectable 87 points made possible by hauls from Tarkowski and Joao Pedro. Too make it even more impressive, he actually also benched 12 points from Anderson.

I bought Konsa three weeks ago and haven't kept a clean sheet. I wouldn't rely on the game against MU.

Now I'm thinking about using my chips to replace the entire team, including him, but I'm not sure when the best time is. I mistakenly sold Haaland and lost a lot of money, but now I see Man City have double gameweeks ahead and I regret selling him.

I was going to get aggressive for this round, but decided against it since I'll need all the FT transfers I can get ahead of the Arsenal / Man City blank next game week...

My main source of headache going into this game week is who to captain, it would have been easier If still have Haaland, now I'm not sure I should go for Semenyo, Ekitike (both could be minutes risk) or stick with Bruno Fernandes.

Don't use chips yet until after GW 31 - use your FT to navigate your way through the upcoming blank week.

You don't necessarily need Halaand, I wanted to sell him this GW before GW31 blank but the Westham fixture is too good to not have him. If you're without Halaand make sure you have Semenyo and then get Pedro + Ekitike/Thiago and you're good to go.
