There are many ways coins might get lost from the wallet and many ways to define what could be called a hack.
That could be a mistake from your side like phishing or password theft, and malware you've unknowingly installed. Even forgetting or erasing the password or 2FA key, private key, mnemonic phrases,  etc results in lost of coins.

Nothing is 100% safe. It's all a question of managing risk. Even having everything offline is a risk in case you lose your private key.
Some exchanges reimburse users who lose money due to an exchange hack, and responsible ones keep the vast majority in cold storage with a minimum in hot wallets.
Coinbase I think is quite good in general, and obviously has a reputation to maintain.

Where you tend not to get reimbursed is if it's a small/disreputable exchange, or anywhere where the mistake is yours - e.g. giving your private key away, software vulnerabilities on your own pc, etc.

I'm using up Coinbase and it isn't easy to bypass it up because anytime you do logged in it do need some SMS verification and other that it do need up some email verification.

Using the Lastpass or keeping the passwords in offline environment are the two best options, from my experience but the obvious problems usually occur in the both online and offline world somehow. Well, hacks hardly happen on decentralized exchanges (real DEXes atleast). Coin exchange on DEXes happen from traders controlled addresses/exchange smart contact. So coin owners are expected to keep their private keys safe

I always use 2FA for all accounts and this is the best security that google offers to all investors when participating in this market. Until now I am fine to use and have not had any problems so please install when you just created a new account. I think if you are still being attacked it is your fault and no one can be held responsible when you have a big loss.

IMO.. if the account that you registered at one of the exchanges is hacked then the one who has the right to be responsible is you yourself because you are not maximally secure your account every transaction, but if many accounts on the exchange are hacked then the developer responsible is the exchange due to security they are very weak

That's not exchange's fault if only the amount from his wallet got stolen. Exchanges can be hacked but with coinbase, it has not happened so OP got hacked via malware installed on his device or his email account got compromised so hacker got the access to his wallet.

Well for me you cannot really blame someone just because they got hacked. Hackers will find a way if they really want to hack your account. The only thing you can do is to maximize the security that you have. If they have a double authenticator, use it to protect all of your coins in your wallet. Actually there are records of some people who got hacked despite enabling the 2FA whether through SMS or app. But I guess right by now all the problems have been fixed and I haven't seen any hacking ever since. I guess it will be mainly your responsibility because it is your account. Upon registering your account it has been your responsibility to take care of it and anything that will happen to it must be taken up into you. On the other hand, the hacker do also have the responsibility because he is the one who have hacked your account just to take it into his own possession also for the sake of his own. With wallets that may be true, but I guess the OP is talking about accounts in cryptocurrency exchanges. If someone hacks the exchange accounts, then you can't just blame the users. The exchanges should also bear a part of the blame. Here, security precautions such as 2FA and email verification are the responsibility of the exchanges. I own accounts in several cryptocurrency exchanges, and till now none of them have got hacked. But some of my friends were victims of such hacks. And they never got any sort of compensation from the exchanges.

Agree, if the event is in the exchanger and occurs on all accounts, then the exchanger should be responsible for compensating the loss suffered by the customer. On the other hand, customers or investors should not keep coins or tokens in the exchanger because they do not hold control over the coins or tokens so that in my opinion it is better to store them in our own wallet
