A guess is all that it would be because we don't know the future difficulty levels.  Future difficulty levels are dependent on cost and availability of ASIC hardware, as well as the future exchange rates.

Just out of curiosity, have you tried using the transaction type "Custom" instead of "Quick Send"?  That way you can select which inputs are used and you can even set the fee to 0 (or whatever value you wish).    That might help to resolve your issue.

No, they don't.  MoneyPak is a product for U.S. only, however in the UK is a similar product, UKash.  A UKash can be used to buy SLLs (which can be traded for BTCs) at VirWoX, or UKash can be used at Mercabit.eu, Bitcoin Nordic, and more.  The fees aren't trivial but this is one way to pay with cash, with no bank system intermediary.

They are frequently "out of stock", but if you monitor their forum thread you can learn when they've restocked.

When I enter a buy order for bitcoin, paying 2% premium I might enter it as:

What I'm suspecting is that you are looking for a dynamic price method, just like how "{mtgoxlast} * 1.02" will dynamically calculate the price based on "market" price.  To provide that, the Gribble bot would need to be modified to become aware of the alt-coin exchanges.  I'm not even sure how you would determine "market price", since unlike bitcoin there isn't one dominant marketplace for each alt coin, as far as I know.  You could make an appeal to get at least some exchange quotes for some alts added to gribble.   Or you could make your own bot (using the open source gribble code) and have a separate OTC channel.

Nope.

Both the parent and child transactions can be included in the same block.  I'm using the term "child" to refer to one which uses an output of a "parent" transaction which also has not yet been included in any prior block.

If I remember correctly, the bitcoin.org client will arrange transactions in the block so that parent's transaction occurs first and child transaction(s) occur later in the block.   I am unsure if a block that has the order reversed would have any problem getting accepted by other nodes if that isn't the case.

The .html from BitAddress contains all the javascript, images, css, etc., so all you need to do is View Source -> Save As (or whatever method your browser uses to save a copy of the .html to disk) and store it to a flash drive. 

Then when you boot to an offline LIveOS, just open the .html from the flash drive. 

If you are using a Linux-based LiveOS, you can probably do a sha1sum and it should be identical to the sha1sum the latest (to verify that the version on the thumb drive matches the one served by BitAddress):

Sounds like you are trying to provide exchange services without sufficient operating capital (cash).  Other fixed-rate exchanges that have made it through that sucky phase have done so after raising capital so that a couple day delay in moving funds doesn't put them out of business.  They still have periods where they can't serve the entire demand, but it isn't a regular occurrance (nor nearly-fatal to them as a business when it does occur).

Maybe it is timing out, or perhaps it is encrypted and the import utility can't accommodate wallets that have passphrase encryption enabled.

You might instead look at trying the Desktop Sync method for blockchain.info, where it grabs the keys from the wallet from your running Bitcoin-Qt.

Or you could export the keys from Bitcoin-Qt (right from the Debug window) and import those into blockchain.info/wallet

A paycheck sent via Direct Deposit is, in the U.S., using the ACH network.  Oftentimes, an employer or payment distribution service will require that the recipient account must be in the employee's name.      So this bitcoin conversion service would need to provide and individual account number for each customer so the account could be in that person's own name.   That isn't all that difficult but the demand today is probably too low to warrant developing such a service to do that.   Additionally, though paychecks from employers are rarely reversed, that doesn't mean they are non-reversible.  So this service would need to charge a fee sufficient to accommodate fraudulent transactions where it delivered bitcoins and at a later time the funds are clawed back for whatever reason (e.g., fraud where it really wasn't a corporate payroll but someone getting a direct deposit sent through fraudulent means.)

Until that day comes that a service like this exists, a service such as Coinbase (in the U.S.) can draw funds from a bank account using the direct deposit/ACH network.   It takes a few days for the coins to be delivered but it is a way that proceeds from a paycheck can be used to buy bitcoins without having to visit a bank or ATM.    

A bank account might not even be needed.  Some prepaid card providers (e.g., Western Union prepaid card and American Express prepaid cards) provide an unique account number and allow Direct Deposit/ACH debits (for loading) and credits (for drawing form the balance).   AMEX BlueBird and Green Dot Go Bank also might have this service, if I remember correctly.  So presumably you can use the card for direct deposit of pay and then have Coinbase draw from the card just as if it were a bank checking account.

Real close.
