Not in my observation over the years.  I've seen it happen when there was a coding bug, user error, or Mt. Gox deciding that a user was suspect, but it is a rare and I've personally had no problems with them in this respect either before or after I was 'verified'.  But I don't do a lot of such transactions and have not done one for months.

When any supposed legitimate organization fails to provide legitimate BTC transactions in a timely manner (including potentially the time they need to go to deep storage) is a noteworthy event and something to watch.  Anyone who has either positive or negative results here would do the community a favor by speaking up such that it can be understood whether the issue is systemic or not.

I think it's not the public faces of mtgox which are responsible for all these issues. All this looks more like a Sword of Damocles in the shape of some gag-order and active supervising by a thirdparty. Don't forget that they are in Japan which might as well make them sit in the middle of the US.

A gag order is a very possible explanation for a lot of the communications failures, but I doubt that one would force them to repeat the 'several weeks' estimates.  Thus, they are either completely lost about how their business is progressing or willing to offend their customers with bald-faced lies.  Neither is very appealing and the honest truth is that I expected better from Mt. Gox.

Companies who are willing to shoot themselves in the foot by producing patently false information often do so because they see the end of the tunnel...and there is no light there.  I honestly hope that Mt. Gox succeeds but it is questionable enough to me at this point that I drew down my exposure in the case of a collapse.  I did the same with Bitcoinica several months before they went down.  It was mostly dumb luck, but if one has their eyes open certain signs can sometimes be noted.

Hello,

Thank you for contacting us. Please be aware that:

When withdrawing or depositing bitcoins into your Mt.Gox account there may occasionally be a delay (1~24 hours) depending on network congestion, your internet connection speed and the time required to get 6 confirmations on the network. If you're sending to your computer's wallet, it's good practice to keep your bitcoin client running so it can download the block chain. For those that may not know the block chain is the complete history of all transactions made on the bitcoin network, and as such it can take some time to update and download the most current block and see the bitcoins in your wallet.

If you aren't seeing the funds after either withdrawing/sending to Mt. Gox from your bitcoin program, please wait 24 hours before contacting us. If your transfer has still not occurred within 48 hours, please lodge a support request so we may begin the process of tracking your bitcoins and reinitiating the transfer.

We apologize for any inconvenience this may cause. If you have any further enquiries, please do not hesitate to contact Mt.Gox Support by responding to this email (should the enquiry relate to this ticket).

Best regards,

Mt.Gox Team

I recently received an email stating that they can "manually" process my transfer for a 5% fee if I am in a hurry for funds

Could you post that email here or did somebody else already?

Wow, they're still using that "network congestion" excuse as if the problem is outside their control, when it's totally their own problem.

Most likely they run out and had to go to cold storage.

How it could possibly be more efficient to field a lot of tickets (and lie to + offend people in the process) than to simply put up a notice is beyond me.

More than that, though, when someone runs out of hot supply it is noteworthy.  It typically means that business is not chipping along in an ordinary manner as anticipated by system design...

Personally I would be perfectly fine being inconvenienced by exhaustion of hot storage from time to time because it means that the provider is being conservative in their risk implementation.  But there is no reason that I can see to hide or lie about the nature of a failure for hot/cold storage reasons or really for any other reason.

Why some people still trust it is beyond me.

(actually it's pretty clear: blind greed, as always. They will get burnt. At this point, after all this time, they will have deserved it.)

Most likely not.  When that happens, you will get an error message.

Most likely there was a problem with the transaction itself (as created by MtGox).  MtGox tries to fix this automagically after a while (a rather long while, 24 hours I think) by double spending on of the inputs, thus invalidating the original transaction.  When the double spending transaction has six confirmations, it will try to send again using new inputs.  This has happened quite often recently.  I haven't figured out what the real bug is.  Their transactions seem perfectly OK to me.

You can fetch all transactions which has gone unconfirmed for more than 2 hours through the API here: https://data.mtgox.com/api/0/bitcoin_tx.php

Some pools, e.g. Eligius, use the coinbase tx to send rewards to miners.  A miner can get the reward sent directly to an exchange, but outputs from the coinbase tx won't be valid as input until it has has at least 100 confirmations.
