Genie is being a Bitmain fan again. Based on my experience and what I've seen in this and other threads during a long time period, the failure rate is far greater than 1%. I would like to see where the number less than one percent is based on.

You can build a miner by buying old Antminer S1, S3 or S5 and then Hotmine X5 or Hotmine X6 upgrade kit from Hotmine. Hotmine: http://en.hotmine.io/x5-x6-upgrade-kit-newWith first two (S1, S3) you need to buy separately a BB board (Beaglebone Black) and build a supporting bracket for it. S5 has BB board. Try connecting that zero hashrate hash board in chain 7 (connector J7 in the controller) to another controller data connector. Try to see if there is red LED light on in the hash board. If the light is off and changing controller data connector makes no difference, and power connections etc. are ok, then you have a dead hash board and it is time for a repair job. On and off sequence with a long pause between is ok. Don't worry.
If for some reason electricity is flickering fast between on and off, then that is a bad thing.

You would need something like a pico psu and then a 5V USB output, doesn't pay the effort and the 5V USB wall wart is a lot cheaper solution.
